Required Documentation
To get started, please have the following information and documents ready:
- MC/MX# or DOT#
- If you’re an intrastate carrier without a DOT#, an applicable state registration number may be provided
- Company Contact Information – Main office phone number, address, and point of contact
- Electronic W-9 – Tax ID information required
- $100,000 Cargo Coverage
- $1,000,000 Auto Liability
- $1,000,000 Commercial General Liability
Carrier Requirements
To qualify, carriers must meet the following standards:
Active Common or Contract Authority
Safety Rating must be Satisfactory
Authority Active for at least 365 days
